A Council on Aging Silicon Valley Care Consultant is available to help you create a care plan customized to your family’s needs. As a member of your team, the Care Consultant is here to:

Listen • Assess • Create • Implement

The Care Consultant is here to listen to you, help you complete a detailed assessment of your circumstances, and provide solutions for all your care problems including implementing appropriate support services.

Because of busy lives caregivers often wait for crisis time to look for help. In a crisis, solutions seem impossible and everything appears to be overwhelming. The Care Consultant’s aim is to alleviate your stress during this uncertain transition, and to ensure you and those you care for maintain independence with dignity.

The Care Consultant will provide you with the peace of mind that you need in order to provide excellent care to your elderly loved one. The majority of all caregivers are wearing a lot of hats; spouse, parent, employee, sister, accountant, chauffer, domestic superhero… We help you achieve a healthy balance.
