Profile
This program is designed for seniors and others who need a coordinated
plan to utilize a variety of community services in order to remain
independent and in their homes. Many Care Management clients are
frail, have recently been discharged from a hospital and/or are
in immediate danger of being placed in a nursing facility. The program
consults with family members and others who may be assisting in
arranging a plan for care.
Steps to Arrange Care
The first step is a detailed assessment of the client in order to
determine which services might be needed. This is performed by Care
Management staff, which includes skilled social services and medical
professionals. The next step is to evaluate the availability of
family members and friends along with volunteers in the community
who might assist the client.
The third step is to review all appropriate community
care programs, many of which are funded by the state and federal
governments, in order to make direct referrals.
Finally, Care Management arranges and monitors (when necessary)
the delivery of these services.
Eligibility
Care Management eligibility may be based on one or more of the following
requirements: age, disability, income and residential status.
Costs and Assistance
This program was originally designed to meet the needs of seniors
who are eligible for Medicaid, Medi-Cal and certified eligible for
immediate placement in either an Immediate Level Care or Skilled
Nursing Care (nursing home) facility. Seniors meeting these criteria
usually qualify for program funding assistance through Medi-Cal.
It is important to note that overall funding from Medi-Cal is limited
and other sources of funding for Care Management and community programs
may be required.
Today care management can be made available to
any senior or disabled person provided that funding, whether through
a public, private or personal source can be arranged. Please let
