Security and companionship for seniors living alone

Program Description
Council on Aging Lifeline offers seniors or anyone living alone an easy, reliable and fast way to call for assistance.

A small console, which attaches to the telephone, links seniors to a 24-hour Support Center where attendants are ready to summon family, friends, and neighbors along with police, fire, and other emergency personnel.

Benefits of Council on Aging Lifeline:

• It costs about $1.13 a day

     • Waterproof, lightweight button

          • No long term commitments

               • No equipment to purchase

                    • Live 24/7 emergency assistance

                         • Local customer support team

Signing Up
Signing up for COA Lifeline is easy as 1, 2, 3...

1. Fill out our downloadable Subscriber Data Form

2. Email, mail, fax or drop the filled out form off

3. And then leave the rest to us.

We will contact you as soon as we receive the form to schedule an appointment within 72 hours (weekends and holidays permitting).

Eligibility
Anyone can reap the benefits from the security of 24-hour monitoring; age is not a factor to us. Our program is available throughout Santa Clara County, and we can provide you with the information for the right organization anywhere in the United States.

If you are already a COA Lifeline subscriber and need to make changes and/or corrections to your service please complete the Change form and email, mail, fax or deliver it to us at the Council on Aging Silicon Valley.
